Your online submission of a personal memoir for our Hurricane Katrina Book can be accepted with your agreement to the following terms and conditions. To accept the terms and conditions, to be bound by them and to gain access to the online submission screen, read the Agreement. If you agree with the terms of the Agreement, select and click on "I Agree" below to submit the Agreement. If you do not agree with the terms of the Agreement select and click "I Don’t Agree" and you will be redirected to the home page.

Your online submission to the Hurricane Katrina Book can be accepted with your agreement to the following terms and conditions. To accept the terms and conditions, to be bound by them and to gain access to the online submission screen, read the agreement and select and click "I Agree", If you agree with the terms of the Agreement. If you do not agree with the terms of the Agreement select and click "I Don’t Agree" and you will be redirected to the home page. If your story is selected you will be notified (via email or hard copy letter). Also, in the event that you are selected, you will be given pertinent information regarding my commitment to the donation of the specified royalty percentage of funds. You must be 19 years of age or have the expressed written consent of an adult to make a submission for inclusion in this project. Due to legal restraints, I cannot donate money directly to survivors as I originally intended. With that in mind, I have pledged 50% of my proceeds from this book to be equally distributed between the following agencies that are working hard to help hurricane survivors (Habitat for Humanity and Oprah Winfrey's Angel Network).

I herby agree to the following:

1. Voluntary Submission.

I voluntarily provide an original account of my experiences during Hurricane

Katrina (written by myself; not a copy of another person’s writing living or

dead) of no cost to Astrya Richard or any of her affiliates in attempts to

produce published work. Astrya Richard nor any of her affiliates will not owe

me any monies, royalties, acknowledgements, gifts, favors now or in the

future for using my story and any contents included with said story.

2. Representation and Contact Information.

I agree to include with my submission the name I wish to appear next to my

work and my pertinent contact/background information (i.e. Name, age, address,

email address, and phone number).

3. Intellectual Property.

Once my story is in the possession of Astrya Richard and her affiliates the

story and all its contents becomes the property of Astrya Richard, for use in

this project. I understand that by submitting my story to Astrya Richard, I am giving

her free liberty to use the story to her free will and I do not hold her liable for any

results for using the story.

4. Term and Termination.

This Agreement will become effective when User clicks on "I Agree" and will

continue in effect thereafter unless severed by Astrya Richard.

5. Legal Adult or Authorized Minor.

I hereby declare that I am at least eighteen years of age or will forward

formal approval from my legal guardian to submit my work in the event that my story

is selected.
